Hematuria

Hematuria is a common urologic condition where blood is present grossly or detected microscopically in the urine. Causes of blood in the urine are many and may include urinary tract infection and/or inflammation, trauma, strenuous exercise, kidney or bladder stones, enlarged prostate gland, kidney cancer, ureteral cancer, bladder cancer, prostate cancer and urethral cancer.

There are also many non urologic causes of hematuria and these include nephrological (such as various medical kidney diseases) and gynecological causes (examples include menstruation, vaginitis, cervical cancer, uterine cancer).

Symptoms

Hematuria may or may not be symptomatic. Some symptoms that may accompany hematuria include:

  • Burning with urination
  • Frequency, urgency of urination
  • Flank pain, back pain, abdominal pain
  • Difficulty urinating
  • Urinary incontinence
  • Urinary retention
Diagnosis

Diagnosis of hematuria is relatively straightforward, especially in cases of gross hematuria where direct visualization of bloody urine establishes the diagnosis. Microscopic hematuria can be identified by microscopically examining the urine.

Once the diagnosis of hematuria is established, a thorough evaluation is necessary to identify the possible causes. This evaluation may include:

  • Cystoscopy: a small camera is introduced via the urethra into the bladder to evaluate for enlarged prostate, inflammation, bladder cancer and other possible causes.
  • Blood test: often performed to evaluate kidney function.
  • Urine cytology: urine is collected and a pathologist evaluates the cells present under the microscope to see if they resemble cancerous cells.
  • Imaging tests: imaging of the kidneys and ureters can be performed with CT or MRI scan to exclude kidney and ureteral cancer.
Treatment

Treatment for hematuria will vary depending on the underlying cause. Some examples include:

  • Infections: infections, including UTIs and STIs, are commonly treated effectively with antibiotics.
  • Kidney stones: some kidney stones may pass on their own, especially those that are small, and others may require surgical intervention.
  • Enlarged prostate: may be treated with prostate medications or surgical intervention depending on degree of hematuria and severity of other urinary symptoms.
  • Urologic cancer: depending on the specific cancer type, treatment may involve surgery, radiation, testosterone suppression, chemotherapy/immunotherapy and ablation, alone or in combination. Please refer to the “Cancer” section of this educational material for additional information.

What is the most serious cause of hematuria?

The most serious cause of blood in the urine is cancer. Cancer of the kidney, ureter, bladder, urethra and sometimes prostate cancer can present with hematuria.
